Your online intake form must be filled out at least 24 hours before your appointment. If you do not fill out the form in advance you may have to be rescheduled under our cancellation policy.
Cancellation Policy
Cancel less than 24 hours before your appointment (or no-show) you will be charged a $25 fee.
Once you have had your visit with the physician and the recommendation has been made our services are non-refundable.
